Mr Manjeet, 22 yrs/Male, met with a RTA on 07 May 2022. He was brought to the casualty of Ujala Cygnus Hospital, Rewari on 08 May 2022 at around 8 pm in an unresponsive condition on ventilatory support. On initial examination his vitals were unstable and his neulogical status was extremely poor with a GCS of E1VetM2. His chances of survival were next to nil. His initial CT scan brain of 07 May 2022 showed left frontal and temporal burst lobe with left FTP ac thin SDH, diffuse cerebral edema, Lt ganglio-capsular bleed and Diffuse Axonal Injury. Repeat CT on 08 May 2022 showed an evolving EDH in left parieto-occipital region apart from the previous findings. Patient was managed under a multidisciplinary Trauma team led by Dr Ajay Prasad, Mch (Neurosurgeon) and Included, Chest Physician, Intensivist, Cardiologist, Physiotherapist, Nutritionist along with a highly professional and dedicated Nursing team. In view of the critical condition of the patient and severe brain injury an emergency decision of urgent brain surgery was taken after explaining condition to Pt’s attenders and taking their consent. Pt was operated upon the same night, emergency Lt FTP decompressive craniectomy with evacuation of ac SDH and EDH was done. Post op the pt was managed on ventilatory support. In view of expected prolonged ventilator support, a tracheostomy was done on 11 May 2022. Post-op Pt developed Ventilator associated pneumonia which was managed with appropriate antibiotics after culture & sensitivity and regular chest physiotherapy. His condition gradually improved and he was slowly taken off ventilator support. His tracheostomy was closed on 18 May 2022. He gradually started following instructions and moving his limbs. Presently he is able to take liquid in sips, sits on chair with support and responds to instructions. He is being discharged to home for further care and is planned for follow up after a week.
